Ipamorelin
Ipamorelin is a synthetic pentapeptide ghrelin mimetic and selective GH secretagogue receptor (GHS-R1a) agonist. Developed in the 1990s, it is considered highly selective for GH release with minimal effects on cortisol, prolactin, or ACTH at therapeutic doses — an advantage over earlier GH secretagogues (GHRP-2, GHRP-6) which stimulate cortisol and prolactin more substantially.
Ipamorelin acts as an agonist at ghrelin receptors (GHS-R1a) on somatotroph cells of the anterior pituitary, stimulating GH release in a dose-dependent fashion. The GH pulse produced is amplified by concurrent GHRH signalling — which is why Ipamorelin is frequently studied in combination with CJC-1295 DAC, Tesamorelin, or Sermorelin. GHS-R1a and GHRH receptors activate complementary intracellular pathways (IP3/PKC and cAMP/PKA respectively), producing synergistic GH pulses exceeding either agent alone.
Research has examined Ipamorelin for GH-axis support, body composition (lean mass preservation with visceral fat reduction), bone mineral density support, cardiac function (GH has direct cardiac receptor activity), and recovery from catabolic states. Combination GHRP + GHRH protocols using Ipamorelin plus a GHRH analog are among the most commonly studied GH-stimulation strategies in preclinical research.
