Hexarelin (Examorelin, EP-23905) is a synthetic hexapeptide GHS-R1a agonist and among the most potent GH secretagogues in the GHRP family on a per-dose basis. Developed by Mediolanum Farmaceutici in the 1990s, its extraordinary potency is partially offset by receptor downregulation with repeated dosing — desensitisation develops more rapidly than with Ipamorelin or GHRP-2.
Hexarelin activates GHS-R1a with the highest intrinsic efficacy of the GHRP class, producing very large acute GH pulses. Unique to Hexarelin within the GHS-R1a agonist class, it also has direct cardiovascular activity at CD36 (scavenger receptor class B member 3) expressed on cardiomyocytes — effects completely independent of GH secretion and potentially relevant to cardiac protection in ischaemia. Cortisol and prolactin stimulation is intermediate between GHRP-6 and Ipamorelin.
Acute GH pulse protocols, GH secretagogue pharmacology, cardiac protection research (Hexarelin has demonstrated cardioprotective effects in ischaemia-reperfusion models independent of GH axis via CD36), body composition, and post-infarction cardiac recovery models.
