ACTH(4-7) Pro-Gly-Pro
Semax is a synthetic heptapeptide (Met-Glu-His-Phe-Pro-Gly-Pro) derived from the ACTH(4-10) fragment of adrenocorticotropic hormone. The MEHF sequence corresponds to ACTH(4-7), while the C-terminal PGP improves metabolic stability. Developed at the Institute of Molecular Genetics of the Russian Academy of Sciences, it is registered in Russia for use in ischaemic stroke, transient ischaemic attack, and cognitive deficit. NA-Semax (N-acetyl Semax) is a modified form with enhanced stability studied for nasal delivery.
Semax markedly increases BDNF (brain-derived neurotrophic factor) and NGF (nerve growth factor) mRNA and protein levels in hippocampal and prefrontal cortical tissue. It also modulates gene expression in the serotonin, dopamine, and GABA systems and activates NO-synthase pathways contributing to cerebroprotection. Unlike ACTH itself, Semax does not activate adrenocortical stress responses due to its truncation at position 7.
Clinical studies in Russia have examined Semax in acute ischaemic stroke (reduction in neurological deficit), perinatal hypoxia, optic nerve disease, and attention deficit disorder. Nootropic applications studied include enhanced sustained attention, working memory under stress, and cognitive performance in healthy subjects. Immunomodulatory effects including regulation of NK cell activity have also been described.
